I consent to the processing of my personal data in the following scope:

• first name, last name, address, telephone number, e-mail address,

• records related to the course of the consultation, personal data shared during communication (e-mails, SMS, messaging applications),

• child’s name, child’s age/date of birth, child’s health status, mother’s health status,

• photographs and/or videos (if taken with consent),

for the purpose of keeping documentation related to the consultation.

The data will be stored for the duration of the consent, but no longer than 10 years.

Consent may be withdrawn at any time by sending a request to the controller’s e-mail address, in writing to the registered office address, or in person. The controller is entitled to verify the identity of the applicant.

Processing without consent (fulfillment of legal obligations):
For invoicing and accounting purposes, the controller processes first name, last name, and address. These data are processed on the basis of a legal obligation, not consent, and are retained for a period of 10 years.

You have the right to:

• access your personal data,

• rectify inaccurate data,

• erase personal data (“the right to be forgotten”),

• restrict processing,

• object to processing,

• data portability,

• withdraw consent at any time, where consent is the legal basis,

• lodge a complaint with the Office for Personal Data Protection (www.uoou.cz).
